Notice is hereby given that by a Deed Poll dated 3 August 2007 and enrolled in the Supreme Court of Judicature on 17 September 2007, CHARLES DAVID LEWIS WARSHAWSKI, 127 Grosvenor Road, Langley Vale, Surrey KT18 6JF, who is married and a Commonwealth/British citizen under section 11(1) of the British Nationality Act 1981, abandoned the name of Charles David Lewis and assumed the name of Charles David Lewis Warshawski.
