I hereby give notice that, by a Deed Poll dated 27 August 2003 and enrolled at the Central Office, Royal Courts of Justice, Strand, London WC2A lLL on 23 October 2003, I, Elaine Everett Johns, of 24 Brentwood Street, Alexander Park Estate, Manchester M16 7LE, the mother of ASHLEY BRITTANY HEARNE, an unmarried child and a Commonwealth citizen, renounced, relinquished and abandoned the name of Ashley Brittany O’Gilvie and assumed the name of Ashley Brittany Hearne. 15 October 2003.
