Notice is hereby given that a Deed Poll dated 30 December 2004 and enrolled in the Supreme Court of Judicature on 13 July 2005, Rachel Hounslow as person having parental responsibility on behalf of the said LEIGH HOUNSLOW, who is a child aged 16, single and a British citizen under section 1(1) of the British Nationality Act 1981, abandoned the name of Leigh Sivyer and assumed the name of Leigh Hounslow.
