Notice is hereby given that a Deed Poll dated 7 September 2004 and enrolled in the Supreme Court of Judicature on 12 October 2004, I, Valerey Camacho Soto and Andres Felipe Ruiz Bejarano, of 31 Bazeley House, Davidge Street, London SE1 0RN, the persons having parental responsibility on behalf of the said MATTHEW RUIZ CAMACHO, who is a child of the age of 6 years, born in the UK granted with Indefinite Leave to remain in November 2003, under section 6(1) of the British Nationality Act 1981, abandoned the name of Matthew Martinez Camacho and assumed the name of Matthew Ruiz Camacho.
