MELIZ MEHMET, of 20 Ravensvale Avenue, North Finchley, London N12 9HS, single adult
and a British citizen by birth under section 1(1) of the British Nationality Act 1981,
renounced relinquished and abandoned the name of Muazzez Mehmed and assumed the name
of Meliz Mehmet.
