Notice is hereby given that by a Deed Poll dated 14 April 2004 and enrolled in the Supreme Court of England and Wales on 18 May 2004, ASHISH BHUPENDRA DAVE, of Krishna Kunjx, Uttoxeter Road, Draycott, Stoke on Trent ST11 9AQ, single and a British citizen, by virtue of section 11(1) of the British Nationality Act 1981, abandoned the name of Aashish Bhupendra Dave and assumed in lieu thereof the name of Ashish Bhupendra Dave.
