Notice is now given that by a Deed Poll dated 18th November 1998 and enrolled in the Central Office of the Supreme Court on 21st March 2001, DAVID CALVIN-RIDGE, of 21 Aisgill Avenue, London W.14, single and a British citizen, by virtue of section 11(1) of the British Nationality Act 1981, formally and absolutely renounced and abandoned the surname of Calvin and has assumed and adopted and intends on all occasions to use and subscribe the surname of Calvin-Ridge instead of the surname Calvin so as to at all times be known and described by the name of David Calvin-Ridge exclusively.
