Notice is now given that by a Deed Poll dated 9th January 2000 and enrolled in the Central Office of the Supreme Court of Justice on 19th May 2000, HUGO CHULA ALEXANDER CHAKRABONGSE, of 32 Chepstow Place, London W2 4TA, single and a British citizen by birth under the British Nationality Act 1981, section 1(1), formally and absolutely renounced and abandoned the surname of Levy and has assumed and adopted and intends on all occasions to use and subscribe the surname of Chakrabongse instead of the surname Levy so as to be at all times called, known and described by the name of Hugo Chula Alexander Chakrabongse exclusively.
