The estate of Jason Chudziak, 10 Lawfield Road, Dalkeith, Midlothian EH22 5AB was sequestrated by the sheriff at Edinburgh on Monday 12th January 2004 and Gillian Thompson, Accountant in Bankruptcy, George House, 126 George Street, Edinburgh EH2 4HH has been appointed by the Court to act as Interim Trustee on the sequestrated estate.

Any Creditor of the debtor named above is invited to submit his statement of claim in the prescribed form, with any supporting accounts or vouchers, to Eric R H Nisbet, The Glen Drummond Partnership, Knightsridge Business Park, 4 Turnbull Way, Livingston EH54 8RB, the agent acting on behalf of the Accountant in Bankruptcy in this sequestration.

For the purpose of formulating claims, creditors should note that the date of sequestration is Monday 12th January 2004.
