CHANGE OF A NAME BY DEED POLL ENROLLED IN CENTRAL OFFICE

Notice is hereby given that by Deed Poll dated the 30th day of January 2006 and enrolled in the Supreme Court of Judicature on the 16th day of February 2006, as Anna Blackwell, having the parental responsibility on behalf of Emily Clare Blackwell, a child, single and a common wealth citizen, abandoned the surname of Hegarty and assumed in lieu thereof the surname of Blackwell.
