TAKE NOTICE that by Deed Poll executed on 30th day of September 2004 and enrolled in the Supreme Court of Judicature in Northern Ireland on the 7th day of October 2004 that Tony Clyde, c/o Stewarts Solicitors, 3 Regent Street, Newtownards, Co Down BT23 4AB, single, absolutely and entirely renounced, relinquished and abandoned the use of his former christian names of Diann Jacqueline and assumed, adopted and determined to take and use the christian name of Tony in substitution for his former christian names of Diann Jacqueline.
