Know all men by these presents (which are intended to be enrolled at the Central Office of the High Court of Justice in Northern Ireland) that I, Bernadino Kim Cowan the undersigned of 11 Glencree Park, Jordanstown, County Antrim, BT37 0QS, single, Civil -Servant, formerly Bernadette Cowan now or lately called Bernadette Kim Cowan by Deed Poll dated 20th October 1998, a Commonwealth Citizen.

WITNESS AND IT IS HEREBY DECLARED as follows: I absolutely and entirely renounce relinquish and abandon the use of my said former forename of Bernadette and assume adopt and determine to take and use the forenames of Bernadino Kim in substitution of my former forenames of Bernadette Kim.

I shall at all times hereinafter in all records, deeds and documents and other writings and in all actions and proceedings as well as in all dealings and transactions and on all occasions whatsoever use and subscribe the said names of Bernadino Kim as my forenames in substitution for my former forenames of Bernadette Kim so relinquished as aforesaid to the intent that I may hereafter be called, known or distinguished not by the former forenames of Bernadette Kim but by the forenames of Bernadino Kim only.

I authorise and require all persons at all times to designate describe and address me by the adopted forenames of Bernadino Kim.
