This notice applies to you if you were a member of the above Scheme (or if you are a widow, widower or other dependant of a member of the Scheme). If you think you are still entitled to benefits under the Scheme, but have not recently been contacted by the Scheme Trustees, please contact us using the address listed below.

If you believe you are still eligible for benefits, please contact Barnett Waddingham as the Scheme Administrators, via Domnick Hunter Administration Team, Barnett Waddingham LLP, 2 London Wall Place, 123 London Wall, London, EC2Y 5AU, or alternatively by email, domnick.hunter@Barnett-Waddingham.co.uk. You should include details of your full name, your current address, National Insurance number, a note of any benefits to which you think you may be entitled and copies of any evidence that you can provide to confirm your entitlement.

Beneficiaries should make claims in writing to the address above not later than 2 months from the date of this notice. The Trustees are in the process of insuring benefits with an insurance company for the members and beneficiaries of the Scheme. Therefore, the Trustees wish to ensure that their records include all members and beneficiaries so that the correct benefits are insured.
