Notice is given that Dr Nadeem Petkar, partner at Crystal Peaks Medical Centre (the "Partnership"), retired from the Partnership with effect from 21 November 2014 and that Dr Anthony Richard Mark Andrzejowski, Dr George Scott Davison, Dr Sarah Costen and Dr Kirsty Gillgrass continued the Partnership under the same partnership name, Crystal Peaks Medical Centre, with effect from 22 November 2014 and which continues to date.
