Any person who believes that he or she has a claim against, or entitlement to any benefit from, or interest in the Plan, which has qualified for assistance under the Financial Assistance Scheme (FAS) and will transfer to FAS, by reason of his or her own or a relatives or any other person’s full or part time employment with any of the companies listed above is required to send particulars in writing by 31 May 2014 of his or her claim or entitlement. You should send full details to the Trustee of the Plan c/o Matthew Whyte, Mercer Limited, 1 Whitehall Quay, Whitehall Road, Leeds, LS1 4HR. You should include your full name, address, date of birth, National Insurance Number, date of entry into membership of the Plan, details of where and by whom you were employed and details of any benefits you think you are entitled to. If you are a widow, widower, civil partner, child or dependant of a former employee then please indicate this on any correspondence and include the requested information in respect of the former employee also. You will also need to provide marriage and birth certificates in due course as proof of entitlement.

The Trustee will transfer the assets of the Plan to FAS who will be responsible for paying the appropriate level of assistance to persons entitled thereto having regard only to those persons of whose claims and entitlements and interests they have notice of and may not be liable for any other person.

If you already receive a pension from the Plan, or you have received in the last year announcements from the Plan, you do not need to write to the Trustee as they already have full details of your entitlements.
