Notice is hereby given that a Deed Poll dated 5 January 2006 and enrolled in the Supreme Court of Judicature on 27 June 2006, Joanne Lesley Elizabeth Slater, of 11 Greavestown Lane, Lea, Preston, Lancashire PR2 1PD, as person having parental responsibility on behalf of the said JOSEPH ROBERT DANIEL SLATER who is a child, single and a British citizen under section 1(1) of the British Nationality Act 1981, abandoned the name of Joseph Robert Daniel Green and assumed the name of Joseph Robert Daniel Slater.
