Notice is hereby given that I, GAVIN JOHN CHAPPELL, of 43 Grasmead Avenue, Leigh-on-Sea, Essex SS9 3LA, formerly called and known by the name of Gavin John Shufflebotham only, a citizen of the United Kingdom by birth, under section 1, subsection 1, give public notice that, on 28th June 2000, I formally and absolutely renounced and abandoned the use of my surname of Shufflebotham only and assumed and adopted in place thereof the surname of Chappell and further that such change of name is evidenced by a Deed Poll dated 28th June 2000 under my hand duly executed by me and attested and enrolled in the Enrolment Department of the Central Office of the Supreme Court of Judicature on 27th November 2000.
