KNOW ALL MEN BY THESE PRESENTS which are intended to be enrolled at the Central Office Of the High Court of Justice in Northern Ireland that I the undersigned Mary Theresa Lavelle Divorced, of 27 Drumreagh Park, Rostrevor, County Down unemployed, now or lately called Mary Theresa Greer a Commonwealth citizen.

WITNESS AND IT IS HEREBY DECLARED AS FOLLOWS:

I absolutely and entirely renounce relinquish and abandon the use of my said former surname of Greer and assume adopt and determine to take and use the surname of Lavelle in substitution for my former surname of Greer.

I shall at all times hereafter in all records deeds, documents and other writings and in all actions and proceedings as well as in all dealings and transactions and on all occasions whatsoever use and subscribe the said name of Lavelle as my surname in substitution for my former surname of Greer so relinquished as aforesaid to the intent that I may hereafter be called, known or distinguished not by the former surname of Greer but by the surname of Lavelle only.

I authorise and require all persons at all times to designate describe and address me by the adopted surname of Lavelle.
