Notice is hereby given by a Deed Poll dated the 21st day of November, 2002 and enrolled in the Supreme Court of Judicature on the 11th day of March 2003, as Pauline Elizabeth Smith as having parental responsibility on behalf of Kirsty Hollie Smith, a child, single and a British Citizen, abandoned the surname of Wells and assumed in lieu thereof a surname of Smith.
