Notice is hereby given that a Deed Poll dated 17 January 2020 and enrolled in the Senior Courts of England and Wales on 19 February 2020, KEELEY ANN AZALAS-GRIFFITHS of 116 Ravenscourt Drive, Basildon, Essex SS16 4HY, single and a British Citizen, under section 1(1) of the British Nationality Act 1981 abandoned the name Keeley Ann Griffiths and assumed the name of Keeley Ann Azalas-Griffiths.
