Notice is hereby given that a Deed Poll dated 5 October 2015 and enrolled in the Senior Courts of England and Wales on 3 December 2015, Hiley Frederica Campbell, 39 Oxford Road, Harrow, Middlesex HA1 4JH, Divorced/Civil Partnership dissolved and a British Citizen under section 11(1) of the British Nationality Act 1981 abandoned the name Hiley Frederica Lewis and assumed the name of Hiley Frederica Campbell.
